Every child under the age of 16 must apply in person for a U.S. passport, with both parents (or legal guardians) present. Child passports are valid for 5 years — they cannot be renewed by mail. When the 5 years are up, the child reapplies as if it were the first time.

A Child U.S. Passport Application is required when:
- The applicant is under 16 years old at the time of application
- Even if the child has previously held a U.S. passport
- Including newborns and infants — there is no minimum age
Children turning 16 or older during the application window should apply as adults using DS-11.

- Child's certified birth certificate — with raised seal, listing both parents
- Both parents' government-issued photo IDs — plus front-and-back photocopies
- Evidence of parental relationship — usually the birth certificate
- Both parents present at the acceptance facility, OR notarized DS-3053 consent from the absent parent
- One passport photo of the child (2x2 requirements) — eyes open, taken within 6 months
