The form you file depends on how recently your passport was issued:
- Less than 1 year ago — file DS-5504. The State Department waives the application fee in this window. (Service and shipping fees still apply.)
- More than 1 year ago — file DS-82 (a regular renewal) with your name-change documentation attached.

Use a U.S. Passport Name Change Application if you've changed your name through:
- Marriage — provide a certified marriage certificate
- Divorce — provide the divorce decree restoring your prior name
- Court order — provide certified court documents
- Naturalization — provide your naturalization certificate
If you're correcting a typo or printing error on a recently-issued passport, that also goes on DS-5504 and is handled the same way.
What you'll need for a Passport Name Change Application
- Your existing passport — must be in your possession and undamaged
- Certified legal document establishing the name change (original or certified copy — not a photocopy)
- One new passport photo (2x2 requirements) — taken within 6 months under your new name
- Completed DS-5504 or DS-82 — we determine which based on issue date and generate it
