If your passport was stolen, the first priority is invalidating the book so it can't be used by someone else. Form DS-64 is the sworn statement that triggers that invalidation in the State Department's system. We file it alongside a new DS-11 (new-passport application) and expedite the replacement through a regional passport agency.
You should also file a police report locally and (if abroad) report the theft to the nearest U.S. embassy or consulate.
Use a Stolen U.S. Passport Application if:
- Your most recent valid passport was stolen — at home, in transit, or after a break-in
- You're in the United States and need a domestic replacement
- You have not yet filed DS-64 (we'll handle it) or have the report number from a prior filing
