Reporting a lost U.S. passport is a two-form process. DS-64 is the sworn statement that officially invalidates the missing book in the State Department's system — no one can use it for travel after that point. DS-11 (new-passport application) is what you file alongside it, in person, at an acceptance facility.
Use a Lost Passport Application if:
- Your most recent valid U.S. passport is lost (you don't know where it is)
- You need a replacement issued in your name
- You're inside the United States — passports lost abroad require an embassy report instead

What you'll need for a Lost U.S. Passport Application
- Proof of U.S. citizenship — birth certificate or naturalization certificate
- Government-issued photo ID (what counts) + photocopy
- One new passport photo (2x2 requirements) — taken within the last 6 months
- Completed DS-11 + DS-64 — we generate both from your online intake
